Who is eligible for the Long-Term Care Managed Care Program?

  • Individuals aged 65 and over and eligible for Medicaid
  • Individuals aged 18 and over and eligible for Medicaid due to disability
  • Be determined by CARES (Comprehensive Assessment and Review for Long-Term Care Services) unit at the Department of Elder Affairs to be at the nursing home level of care and meet one or more established clinical criteria
